Hiring Forms
Teaching:
- Requisition and Hiring Form (Cont. Ed.)
This form needs to be completed to start the posting and hiring process for Continuing Education positions.
Non-teaching:
Payroll Forms
- Staff Timesheet (Overtime and Hourly Paid Timesheet)
This form must be completed and signed by both the manager and the employee and forwarded to the Department of Administrative Services, before the payroll deadline. A hiring requisition must either accompany it or have been submitted previously. - Student Timesheet
This form must be completed and signed by both the manager and the student employee and forwarded to the Department of Administrative Services, before the payroll deadline. A hiring requisition must either accompany it or have been submitted previously for work done in that semester. - TD1 – Personal Tax Credit Return (Revenue Canada) & TP-1015.3V – Source Deductions Return (Revenu Quebec)
These forms are used to determine the amount of tax to be deducted from an individual’s employment income.
Examples of who should be completing these forms – individuals who:- have a new employer;
- want to change amounts previously claimed;
- want to increase the amount of tax deducted at source;
